Transitions LifeCare
About
Transitions LifeCare is a nonprofit hospice and palliative care provider based on Hospice Circle in Raleigh. The staff offer guidance, resources and care to people at any stage of a life-changing illness, so they can live as fully as they wish.
It began in 1979 as Hospice of Wake County, with a single staff member and 20 volunteers. The board approved the current name in 2013, once the work had grown past hospice alone and past the county line. Much of that growth came from local giving.
